Allergies
Speak with the school nurse regarding any learner’s allergies. This includes any dietary allergies and/or special dietary needs. Notify the school nurse if the allergy changes in severity or is no longer a concern.
Each year, prior to the first day of school, provide the following items to the school nurse if the learner’s allergy requires possible emergency medication:
- Emergency Care Plan
- Medication Administration Authorization Form. This must be signed by a medical provider. For additional information, see the Medication page.
Dietary Allergies/Special Dietary Needs
- The Dietary Accommodation Form will need to be completed and signed by a medical provider once for your child. The only time this would need to be completed more than once is if an additional allergy develops. It is the responsibility of the parent/guardian to notify the school nurse of any changes.
The school nurse will share this information on a need-to-know basis with school staff such as teachers, clerical staff, bus drivers, and other support staff.
